Executive Summary

The US Department of Health and Human Services (HHS) is to set up a user-friendly internet-based record of medical mistakes made by doctors and hospitals, with the intention of helping them avoid such errors in the future. The database will enable faster cross-matching and electronic analysis of data, and more rapid responses to patient safety problems, the HHS said. It follows similar moves in the UK to record medical errors so that lessons can be learnt from them (Clinica No 954, p 8).
