Executive Summary
Point-of-care medicine in the UK made a further leap yesterday, with the publication of the NHS Long Term Conditions Model. By March 2007, the NHS should have in place 3,000 new so-called "community matrons" who will spearhead out-of-hospital care for the 17.5 million people with one or more long-term conditions. This initiative will be backed by a National Service Framework for long-term conditions, due to published later this year.
